Haleiwa Catastrophic Flooding: City Needs Input to Get Federal Funds
Yesterday, the popular tourist destination – the town of Haleiwa on the north shore of the island of Oahu – was struck by massive flooding, the likes of which long-time residents have never seen before. One person was swept away in a surging river and has yet to be found. Damage to homes and businesses is catastrophic.
The City and County of Honolulu has developed an online form that allows Oahu residents to self-report damages to their homes and/or businesses. The purpose of this form is only to collect information that will help local officials understand the damage that occurred and impacts on the community. The City will use this information to determine whether the City can request federal assistance programs.
This form is NOT an application for relief programs.

- Do not use this form to report immediate, life-threatening needs. Call 911.
- Reporting damage is a voluntary activity and is not a substitute for reporting damage to your insurance agency, nor does it guarantee disaster relief assistance.
- Please submit one survey per affected housing unit or business.
- This form is for damages that occurred on Oahu only.
